Uncertain significance for Cardiovascular phenotype — the classification assigned by Ambry Genetics to NM_174936.4(PCSK9):c.341T>C (p.Val114Ala), citing Ambry Variant Classification Scheme 2023. This variant lies in the PCSK9 gene (transcript NM_174936.4) at coding-DNA position 341, where T is replaced by C; at the protein level this means replaces valine at residue 114 with alanine — a missense variant. Submitter rationale: The p.V114A variant (also known as c.341T>C), located in coding exon 2 of the PCSK9 gene, results from a T to C substitution at nucleotide position 341. The valine at codon 114 is replaced by alanine, an amino acid with similar properties. This variant has been detected in an individual from a hypocholesterolemic cohort and in two individuals whose mean total cholesterol and LDL cholesterol levels were not lower than non-carriers (Fasano T et al. Arterioscler Thromb Vasc Biol, 2007 Mar;27:677-81; Lacaze P et al. Open Heart, 2021 Jul;8). This variant was reported to co-occur in cis with another PCSK9 variant (p.R104C) in a proband with hypobetalipoproteinemia, very low LDL cholesterol, and undetectable circulating plasma PCSK9. Two relatives with both variants were less severely affected but also had hypocholesterolemia and reduced circulating PCSK9 (Cariou B et al. Arterioscler Thromb Vasc Biol, 2009 Dec;29:2191-7). Functional studies of both variants together suggested impact to protein function consistent with dominant negative or loss of function, but results for the individual variants were less significant (Cariou B et al. Arterioscler Thromb Vasc Biol, 2009 Dec;29:2191-7; Si-Tayeb K et al. Dis Model Mech, 2016 Jan;9:81-9; Roudaut M et al. Stem Cell Reports, 2021 Dec;16:2958-2972). This amino acid position is not well conserved in available vertebrate species. In addition, this alteration is predicted to be tolerated by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear.
