Uncertain significance for Desmin-related myofibrillar myopathy — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_001927.4(DES):c.1244G>A (p.Arg415Gln), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces arginine, which is basic and polar, with glutamine, which is neutral and polar, at codon 415 of the DES protein (p.Arg415Gln). This variant also falls at the last nucleotide of exon 6, which is part of the consensus splice site for this exon. The frequency data for this variant in the population databases is considered unreliable, as metrics indicate poor data quality at this position in the gnomAD database. This missense change has been observed in individual(s) with DES-related conditions (PMID: 26431784, 31983221, 33662488, 36264615, 37652022, 38727660). ClinVar contains an entry for this variant (Variation ID: 626714). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) has been performed for this missense variant. However, the output from this modeling did not meet the statistical confidence thresholds required to predict the impact of this variant on DES protein function. Experimental studies have shown that this missense change affects DES function (PMID: 38727660). Variants that disrupt the consensus splice site are a relatively common cause of aberrant splicing (PMID: 17576681, 9536098). Algorithms developed to predict the effect of sequence changes on RNA splicing suggest that this variant may disrupt the consensus splice site. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
