Uncertain significance for Colorectal cancer, susceptibility to, 12 — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NC_000012.12:g.(?_132687244)_(132687325_?)del, citing Invitae Variant Classification Sherloc (09022015): This variant is a gross deletion of the genomic region encompassing exon 1 of the POLE gene, which includes the initiator codon. The 5' end of this event is unknown as it extends beyond the assayed region for this gene and therefore may encompass additional genes. The 3' boundary is likely confined to intron 1 of the POLE gene. This is expected to result in an absent or disrupted protein product. Similar deletions have not been reported in the literature in individuals with POLE-related disease. Experimental studies and prediction algorithms are not available for this variant, and the functional significance of the affected amino acid(s) is currently unknown. Missense variants that disrupt the 3'-5' exonuclease (proof-reading) activity of the POLE protein, while not abolishing its polymerase enzyme activity, are associated with an increased risk for colonic adenomatous polyps and colon cancer (PMID: 23263490, 23447401). Loss-of-function variants, which result in an absent or severely disrupted POLE protein, are therefore unlikely to be associated with disease. Without further clinical and genetic evidence, however, this variant has been classified as a Variant of Uncertain Significance.
