Uncertain significance for Familial hypobetalipoproteinemia 1; Hypercholesterolemia, autosomal dominant, type B — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_000384.3(APOB):c.12457C>A (p.Gln4153Lys), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the APOB gene (transcript NM_000384.3) at coding-DNA position 12457, where C is replaced by A; at the protein level this means replaces glutamine at residue 4153 with lysine — a missense variant. Submitter rationale: In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. This sequence change replaces glutamine with lysine at codon 4153 of the APOB protein (p.Gln4153Lys). The glutamine residue is weakly conserved and there is a small physicochemical difference between glutamine and lysine. This variant is not present in population databases (ExAC no frequency). This variant has not been reported in the literature in individuals with APOB-related disease. Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be tolerated, but these predictions have not been confirmed by published functional studies and their clinical significance is uncertain.
