Uncertain significance for Neuronopathy, distal hereditary motor, autosomal recessive 5 — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_006736.6(DNAJB2):c.71G>A (p.Arg24Gln), citing Invitae Variant Classification Sherloc (09022015): In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. Algorithms developed to predict the effect of missense changes on protein structure and function do not agree on the potential impact of this missense change (SIFT: "Deleterious"; PolyPhen-2: "Possibly Damaging"; Align-GVGD: "Class C0"). This variant has not been reported in the literature in individuals with DNAJB2-related disease. The frequency data for this variant in the population databases is considered unreliable, as metrics indicate poor data quality at this position in the ExAC database. This sequence change replaces arginine with glutamine at codon 24 of the DNAJB2 protein (p.Arg24Gln). The arginine residue is highly conserved and there is a small physicochemical difference between arginine and glutamine.
