Pathogenic for Tuberous sclerosis 2 — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_000548.5(TSC2):c.683dup (p.Tyr228Ter), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the TSC2 gene (transcript NM_000548.5) at coding-DNA position 683, duplicating one base; at the protein level this means converts the codon for tyrosine at residue 228 into a premature stop signal — a nonsense variant expected to truncate the protein. Submitter rationale: This sequence change creates a premature translational stop signal (p.Tyr228*) in the TSC2 gene. It is expected to result in an absent or disrupted protein product. For these reasons, this variant has been classified as Pathogenic. Loss-of-function variants in TSC2 are known to be pathogenic (PMID: 10205261, 17304050). This variant has not been reported in the literature in individuals with TSC2-related disease. However, a different variant (c.684C>A) giving rise to the same protein effect observed here (p.Tyr228*) has been reported in an individual affected with tuberous sclerosis complex (PMID: 21811971). This variant is not present in population databases (ExAC no frequency).
