NM_000061.3(BTK):c.1578C>A (p.Asn526Lys) was classified as Uncertain significance for X-linked agammaglobulinemia with growth hormone deficiency by Labcorp Genetics (formerly Invitae), Labcorp, citing Invitae Variant Classification Sherloc (09022015): This variant has been observed in individuals affected with X-linked agammaglobulinaemia (PMID: 27593100, 7711734). Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be disruptive, but these predictions have not been confirmed by published functional studies and their clinical significance is uncertain. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. This variant is not present in population databases (ExAC no frequency). This sequence change replaces asparagine with lysine at codon 526 of the BTK protein (p.Asn526Lys). The asparagine residue is highly conserved and there is a moderate physicochemical difference between asparagine and lysine.
