Uncertain significance for Hyperaldosteronism, familial, type IV; Idiopathic generalized epilepsy — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_021098.3(CACNA1H):c.4072G>A (p.Ala1358Thr), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the CACNA1H gene (transcript NM_021098.3) at coding-DNA position 4072, where G is replaced by A; at the protein level this means replaces alanine at residue 1358 with threonine — a missense variant. Submitter rationale: In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be tolerated, but these predictions have not been confirmed by published functional studies and their clinical significance is uncertain. This variant has not been reported in the literature in individuals with CACNA1H-related disease. This variant is present in population databases (rs764727820, ExAC 0.006%). This sequence change replaces alanine with threonine at codon 1358 of the CACNA1H protein (p.Ala1358Thr). The alanine residue is weakly conserved and there is a small physicochemical difference between alanine and threonine.
