Uncertain significance for Rolandic epilepsy, intellectual disability, and speech dyspraxia, X-linked — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_014467.3(SRPX2):c.1313A>T (p.Glu438Val), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces glutamic acid with valine at codon 438 of the SRPX2 protein (p.Glu438Val). The glutamic acid residue is highly conserved and there is a moderate physicochemical difference between glutamic acid and valine. This variant is not present in population databases (ExAC no frequency). This variant has not been reported in the literature in individuals with SRPX2-related disease. Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be disruptive, but these predictions have not been confirmed by published functional studies and their clinical significance is uncertain. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
