Uncertain significance for Hypertrophic cardiomyopathy — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_000257.4(MYH7):c.578A>T (p.Gln193Leu), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the MYH7 gene (transcript NM_000257.4) at coding-DNA position 578, where A is replaced by T; at the protein level this means replaces glutamine at residue 193 with leucine — a missense variant. Submitter rationale: This sequence change replaces glutamine with leucine at codon 193 of the MYH7 protein (p.Gln193Leu). The glutamine residue is highly conserved and there is a moderate physicochemical difference between glutamine and leucine. This variant is not present in population databases (ExAC no frequency). This variant has not been reported in the literature in individuals with MYH7-related disease. This variant is found within a region of MYH7 between codons 181 and 937 that contains the majority of the myosin head domain. Missense variants in this region have been shown to be significantly overrepresented in individuals with hypertrophic cardiomyopathy (PMID: 27532257). In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. A computational algorithm designed to assess the pathogenicity of variants in MYH7 with regard to hypertrophic cardiomyopathy predicted this sequence change to be deleterious. The algorithm has a sensitivity of 94% and a specificity of 89% (PMID: 21310275).
