Uncertain significance for Congenital myasthenic syndrome 17; Cenani-Lenz syndactyly syndrome; Sclerosteosis 2 — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_002334.4(LRP4):c.851G>A (p.Arg284His), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the LRP4 gene (transcript NM_002334.4) at coding-DNA position 851, where G is replaced by A; at the protein level this means replaces arginine at residue 284 with histidine — a missense variant. Submitter rationale: This sequence change replaces arginine with histidine at codon 284 of the LRP4 protein (p.Arg284His). The arginine residue is highly conserved and there is a small physicochemical difference between arginine and histidine. This variant is present in population databases (rs369307795, ExAC 0.009%). This variant has not been reported in the literature in individuals affected with LRP4-related conditions. Algorithms developed to predict the effect of missense changes on protein structure and function output the following: SIFT: "Deleterious"; PolyPhen-2: "Benign"; Align-GVGD: "Class C0". The histidine amino acid residue is found in multiple mammalian species, which suggests that this missense change does not adversely affect protein function. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
