Uncertain significance for Myasthenic syndrome, congenital, 22 — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_001171613.2(PREPL):c.1610A>G (p.Tyr537Cys), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces tyrosine with cysteine at codon 626 of the PREPL protein (p.Tyr626Cys). The tyrosine residue is weakly conserved and there is a large physicochemical difference between tyrosine and cysteine. This variant is present in population databases (rs761490421, ExAC 0.003%). This variant has not been reported in the literature in individuals affected with PREPL-related conditions. Algorithms developed to predict the effect of missense changes on protein structure and function output the following: SIFT: "Tolerated"; PolyPhen-2: "Benign"; Align-GVGD: "Class C0". The cysteine amino acid residue is found in multiple mammalian species, which suggests that this missense change does not adversely affect protein function.
