Likely pathogenic for STAT3 gain of function; Hyper-IgE recurrent infection syndrome 1, autosomal dominant — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_139276.3(STAT3):c.307C>T (p.Arg103Trp), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces arginine, which is basic and polar, with tryptophan, which is neutral and slightly polar, at codon 103 of the STAT3 protein (p.Arg103Trp). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individuals with clinical features of STAT3-related immunodeficiency (PMID: 27379089, 35753512, 36228738; internal data). ClinVar contains an entry for this variant (Variation ID: 566510). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) has been performed for this missense variant. However, the output from this modeling did not meet the statistical confidence thresholds required to predict the impact of this variant on STAT3 protein function. In summary, the currently available evidence indicates that the variant is pathogenic, but additional data are needed to prove that conclusively. Therefore, this variant has been classified as Likely Pathogenic.
