Uncertain significance for Progressive myoclonic epilepsy type 8 — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_021267.5(CERS1):c.181C>A (p.Leu61Met), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces leucine with methionine at codon 61 of the CERS1 protein (p.Leu61Met). The leucine residue is moderately conserved and there is a small physicochemical difference between leucine and methionine. While this variant is not present in population databases, the frequency information is unreliable, as metrics indicate poor data quality at this position in the ExAC database. This variant has not been reported in the literature in individuals with CERS1-related disease. Algorithms developed to predict the effect of missense changes on protein structure and function do not agree on the potential impact of this missense change (SIFT: "Tolerated"; PolyPhen-2: "Possibly Damaging"; Align-GVGD: "Class C0"). In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
