Uncertain significance for Developmental and epileptic encephalopathy, 27; Intellectual disability, autosomal dominant 6 — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_000834.5(GRIN2B):c.3202G>A (p.Val1068Ile), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces valine with isoleucine at codon 1068 of the GRIN2B protein (p.Val1068Ile). The valine residue is highly conserved and there is a small physicochemical difference between valine and isoleucine. This variant is present in population databases (rs749434579, ExAC 0.001%). This variant has not been reported in the literature in individuals with GRIN2B-related disease. Algorithms developed to predict the effect of missense changes on protein structure and function do not agree on the potential impact of this missense change (SIFT: "Tolerated"; PolyPhen-2: "Probably Damaging"; Align-GVGD: "Class C0"). In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
