Uncertain significance for Developmental and epileptic encephalopathy, 27; Intellectual disability, autosomal dominant 6 — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_000834.5(GRIN2B):c.2510T>C (p.Ile837Thr), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the GRIN2B gene (transcript NM_000834.5) at coding-DNA position 2510, where T is replaced by C; at the protein level this means replaces isoleucine at residue 837 with threonine — a missense variant. Submitter rationale: In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. Algorithms developed to predict the effect of missense changes on protein structure and function do not agree on the potential impact of this missense change (SIFT: "Deleterious"; PolyPhen-2: "Probably Damaging"; Align-GVGD: "Class C0"). This variant has not been reported in the literature in individuals with GRIN2B-related disease. This variant is not present in population databases (ExAC no frequency). This sequence change replaces isoleucine with threonine at codon 837 of the GRIN2B protein (p.Ile837Thr). The isoleucine residue is highly conserved and there is a moderate physicochemical difference between isoleucine and threonine.
