Pathogenic for Hereditary cancer-predisposing syndrome — the classification assigned by Ambry Genetics to NM_007294.4(BRCA1):c.5277+1G>C, citing Ambry Variant Classification Scheme 2023: The c.5277+1G>C intronic pathogenic mutation results from a G to C substitution one nucleotide after coding exon 18 of the BRCA1 gene. Alterations that disrupt the canonical splice site are expected to result in aberrant splicing. In silico splice site analysis predicts that this alteration will weaken the native splice donor site. A resulting transcript is predicted to be in-frame and is not expected to trigger nonsense-mediated mRNAdecay; although, direct evidence is unavailable. However, the region predicted to be impacted is critical for protein function (Ambry internal data). One functional study found that this nucleotide substitution is non-functional in a high throughput genome editing haploid cell survival assay (Findlay GM et al. Nature, 2018 Oct;562:217-222). This variant is considered to be rare based on population cohorts in the Genome Aggregation Database (gnomAD). This nucleotide position is highly conserved in available vertebrate species. Based on the supporting evidence, this variant is interpreted as a disease-causing mutation.
