Pathogenic for Spongy degeneration of central nervous system — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_000049.4(ASPA):c.548C>A (p.Pro183His), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the ASPA gene (transcript NM_000049.4) at coding-DNA position 548, where C is replaced by A; at the protein level this means replaces proline at residue 183 with histidine — a missense variant. Submitter rationale: This sequence change replaces proline, which is neutral and non-polar, with histidine, which is basic and polar, at codon 183 of the ASPA protein (p.Pro183His). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with ASPA-related conditions (PMID: 10407784, 26586007). In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant. ClinVar contains an entry for this variant (Variation ID: 554010). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is expected to disrupt ASPA protein function with a positive predictive value of 95%. For these reasons, this variant has been classified as Pathogenic.
