Submitter rationale: This sequence change replaces serine, which is neutral and polar, with alanine, which is neutral and non-polar, at codon 399 of the DBT protein (p.Ser399Ala). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with maple syrup urine disease (PMID: 27243974). In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant. ClinVar contains an entry for this variant (Variation ID: 551600). This variant disrupts the p.Ser399 amino acid residue in DBT. Other variant(s) that disrupt this residue have been determined to be pathogenic (PMID: 30228974). This suggests that this residue is clinically significant, and that variants that disrupt this residue are likely to be disease-causing. For these reasons, this variant has been classified as Pathogenic.
