This variant lies in the PKHD1 gene (transcript NM_138694.4) at the canonical splice acceptor site of the intron immediately before coding-DNA position 53, where G is replaced by A; at the protein level this means a change at this position may disrupt normal splicing. Submitter rationale: Variant summary: PKHD1 c.53-1G>A is located in a canonical splice-site and is predicted to affect mRNA splicing resulting in a significantly altered protein due to either exon skipping, shortening, or inclusion of intronic material. Several computational tools predict a significant impact on normal splicing: Two predict the variant abolishes a 5 splicing donor site. Four predict the variant abolishes a 3 acceptor site. However, these predictions have yet to be confirmed by functional studies. The variant allele was found at a frequency of 8e-06 in 251300 control chromosomes (gnomAD). To our knowledge, no occurrence of c.53-1G>A in individuals affected with Polycystic Kidney and Hepatic Disease and no experimental evidence demonstrating its impact on protein function have been reported. Two ClinVar submitters (evaluation after 2014) cite the variant as likely pathogenic. Based on the evidence outlined above, the variant was classified as likely pathogenic.
