Pathogenic for Pyruvate dehydrogenase E3 deficiency — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_000108.5(DLD):c.1446_1447del (p.Arg482fs), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the DLD gene (transcript NM_000108.5) at coding-DNA position 1446 through coding-DNA position 1447, deleting 2 bases; at the protein level this means shifts the reading frame starting at arginine residue 482, producing a truncated or aberrant protein — a frameshift variant. Submitter rationale: For these reasons, this variant has been classified as Pathogenic. This variant disrupts a region of the DLD protein in which other variant(s) (p.Arg495Gly) have been determined to be pathogenic (PMID: 8968745, 21930696). This suggests that this is a clinically significant region of the protein, and that variants that disrupt it are likely to be disease-causing. This variant has not been reported in the literature in individuals affected with DLD-related conditions. This variant is not present in population databases (gnomAD no frequency). This sequence change creates a premature translational stop signal (p.Arg482Serfs*13) in the DLD gene. While this is not anticipated to result in nonsense mediated decay, it is expected to disrupt the last 28 amino acid(s) of the DLD protein.
