NM_000404.4(GLB1):c.1481G>T (p.Gly494Val) was classified as Pathogenic for Mucopolysaccharidosis, MPS-IV-B; GM1 gangliosidosis by Labcorp Genetics (formerly Invitae), Labcorp, citing Invitae Variant Classification Sherloc (09022015). This variant lies in the GLB1 gene (transcript NM_000404.4) at coding-DNA position 1481, where G is replaced by T; at the protein level this means replaces glycine at residue 494 with valine — a missense variant. Submitter rationale: This sequence change replaces glycine, which is neutral and non-polar, with valine, which is neutral and non-polar, at codon 494 of the GLB1 protein (p.Gly494Val). The frequency data for this variant in the population databases is considered unreliable, as metrics indicate poor data quality at this position in the gnomAD database. This missense change has been observed in individual(s) with GM1-gangliosidosis (PMID: 20920281, 30267299). In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant. ClinVar contains an entry for this variant (Variation ID: 550166). An algorithm developed to predict the effect of missense changes on protein structure and function (PolyPhen-2) suggests that this variant is likely to be disruptive. Experimental studies have shown that this missense change affects GLB1 function (PMID: 20920281). This variant disrupts the p.Gly494 amino acid residue in GLB1. Other variant(s) that disrupt this residue have been observed in individuals with GLB1-related conditions (PMID: 16941474), which suggests that this may be a clinically significant amino acid residue. For these reasons, this variant has been classified as Pathogenic.
