Uncertain significance for Hereditary cancer-predisposing syndrome — the classification assigned by Ambry Genetics to NM_006231.4(POLE):c.2932dup (p.Glu978fs), citing Ambry Variant Classification Scheme 2023: The c.2932dupG variant, located in coding exon 25 of the POLE gene, results from a duplication of G at nucleotide position 2932, causing a translational frameshift with a predicted alternate stop codon (p.E978Gfs*6). This alteration is expected to result in loss of function by premature protein truncation or nonsense-mediated mRNA decay. Loss-of-function variants subject to nonsense mediated decay (NMD) in POLE are known to cause POLE deficiency; however, such associations with POLE-related polymerase proofreading-associated polyposis (PPAP) have not been reported. Based on the supporting evidence, this alteration is pathogenic for POLE deficiency; however, the association of this alteration with POLE-related PPAP is unknown.
