NM_174936.4(PCSK9):c.1792G>A (p.Ala598Thr) was classified as Uncertain Significance for Hypercholesterolemia, autosomal dominant, 3 by All of Us Research Program, National Institutes of Health, citing ACMG Guidelines, 2015. This variant lies in the PCSK9 gene (transcript NM_174936.4) at coding-DNA position 1792, where G is replaced by A; at the protein level this means replaces alanine at residue 598 with threonine — a missense variant. Submitter rationale: This missense variant replaces alanine with threonine at codon 598 of the PCSK9 protein. Computational prediction suggests that this variant may not impact protein structure and function (internally defined REVEL score threshold <= 0.5, PMID: 27666373). Splice site prediction tools suggest that this variant may not impact RNA splicing. To our knowledge, functional studies have not been performed for this variant. This variant has been reported in an individual affected with myocardial infarction (PMID: 30971288). This variant has been identified in 26/262780 chromosomes in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance.

This study involves interpretation of variants in research participants for the purpose of population health screening. Participant phenotype was not available at the time of variant classification. Additional details can be found in publication PMID: 35346344, PMCID: PMC8962531
