Pathogenic for Hereditary cancer-predisposing syndrome — the classification assigned by Ambry Genetics to NM_007294.4(BRCA1):c.321del (p.Phe107fs), citing Ambry Variant Classification Scheme 2023. This variant lies in the BRCA1 gene (transcript NM_007294.4) at coding-DNA position 321, deleting one base; at the protein level this means shifts the reading frame starting at phenylalanine residue 107, producing a truncated or aberrant protein — a frameshift variant. Submitter rationale: The c.321delT pathogenic mutation, located in coding exon 5 of the BRCA1 gene, results from a deletion of one nucleotide at nucleotide position 321, causing a translational frameshift with a predicted alternate stop codon (p.F107Lfs*12). This alteration was identified amongst a cohort of 207 Japanese ovarian cancer patients (Sugino K et al. Sci Rep, 2019 11;9:17808). This variant is considered to be rare based on population cohorts in the Genome Aggregation Database (gnomAD). In addition to the clinical data presented in the literature, this alteration is expected to result in loss of function by premature protein truncation or nonsense-mediated mRNA decay. As such, this alteration is interpreted as a disease-causing mutation.
