Likely pathogenic — the classification assigned by GeneDx to NM_001846.4(COL4A2):c.2569G>A (p.Gly857Arg), citing GeneDx Variant Classification (06012015): The G857R variant in the COL4A2 gene has not been reported previously as a pathogenic variant, nor as a benign variant, to our knowledge. The G857R variant is not observed in large population cohorts (Lek et al., 2016). The G857R variant is a non-conservative amino acid substitution, which is likely to impact secondary protein structure as these residues differ in polarity, charge, size and/or other properties. This variant results in the substitution of a Glycine residue in the canonical Gly-X-Y repetitive motif of the triple helical region of the COL4A2 gene. In-silico analyses, including protein predictors and evolutionary conservation, support a deleterious effect. We interpret G857R as a likely pathogenic variant.
