NM_001999.4(FBN2):c.7559G>A (p.Gly2520Glu) was classified as Uncertain significance by GeneDx, citing GeneDx Variant Classification (06012015). This variant lies in the FBN2 gene (transcript NM_001999.4) at coding-DNA position 7559, where G is replaced by A; at the protein level this means replaces glycine at residue 2520 with glutamic acid — a missense variant. Submitter rationale: A variant of uncertain significance has been identified in the FBN2 gene. The G2520E variant has not been published as pathogenic or been reported as benign to our knowledge. This variant is not observed at a significant frequency in large population cohorts (Lek et al., 2016). The G2520E variant is a non-conservative amino acid substitution, which is likely to impact secondary protein structure as these residues differ in polarity, charge, size and/or other properties. In-silico analyses, including protein predictors and evolutionary conservation, support a deleterious effect. However, while the G2520E variant is located within a calcium-binding EGF-like domain of the FBN2 gene, it does not affect a cysteine residue in this domain. Cysteine substitutions in the calcium-binding EGF-like domains represent the majority of pathogenic missense changes associated with congenital contractural arachnodactyly (Collod-Beroud et al., 2003; Frédéric et al., 2009). Therefore, based on the currently available information, it is unclear whether this variant is pathogenic or rare benign.
