NM_000059.4(BRCA2):c.4974del (p.Gln1658fs) was classified as Pathogenic by GeneDx, citing GeneDx Variant Classification (06012015). This variant lies in the BRCA2 gene (transcript NM_000059.4) at coding-DNA position 4974, deleting one base; at the protein level this means shifts the reading frame starting at glutamine residue 1658, producing a truncated or aberrant protein — a frameshift variant. Submitter rationale: This deletion of one nucleotide in BRCA2 is denoted c.4974delG at the cDNA level and p.Gln1658HisfsX12 (Q1658HfsX12) at the protein level. This deletion would be defined as BRCA2 5202delG using alternate nomenclature. The normal sequence, with the base that is deleted in brackets, is ATCA[delG]TCCC. The deletion causes a frameshift which changes a Glutamine to a Histidine at codon 1658, and creates a premature stop codon at position 12 of the new reading frame. Although this variant has not, to our knowledge, been reported in the literature, it is predicted to cause loss of normal protein function through either protein truncation or nonsense-mediated mRNA decay. We consider this variant to be pathogenic.
