NM_007294.4(BRCA1):c.1823del (p.Lys608fs) was classified as Pathogenic for Hereditary cancer-predisposing syndrome by Ambry Genetics, citing Ambry Variant Classification Scheme 2023. This variant lies in the BRCA1 gene (transcript NM_007294.4) at coding-DNA position 1823, deleting one base; at the protein level this means shifts the reading frame starting at lysine residue 608, producing a truncated or aberrant protein — a frameshift variant. Submitter rationale: The c.1823delA pathogenic mutation, located in coding exon 9 of the BRCA1 gene, results from a deletion of one nucleotide at nucleotide position 1823, causing a translational frameshift with a predicted alternate stop codon (p.K608Rfs*4). This alteration was identified in at least one Korean breast cancer patient undergoing BRCA1/2 genetic testing (Ahn SH et al. Cancer Lett, 2007 Jan;245:90-5; Ryu JM et al. Breast Cancer Res Treat, 2019 Jan;173:385-395). This variant is considered to be rare based on population cohorts in the Genome Aggregation Database (gnomAD). In addition to the clinical data presented in the literature, this alteration is expected to result in loss of function by premature protein truncation or nonsense-mediated mRNA decay. As such, this alteration is interpreted as a disease-causing mutation.
