Uncertain significance for Myasthenic syndrome, congenital, 2a, slow-channel — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_000747.3(CHRNB1):c.257A>G (p.Tyr86Cys), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the CHRNB1 gene (transcript NM_000747.3) at coding-DNA position 257, where A is replaced by G; at the protein level this means replaces tyrosine at residue 86 with cysteine — a missense variant. Submitter rationale: This sequence change replaces tyrosine with cysteine at codon 86 of the CHRNB1 protein (p.Tyr86Cys). The tyrosine residue is moderately conserved and there is a large physicochemical difference between tyrosine and cysteine. This variant is not present in population databases (ExAC no frequency). This variant has not been reported in the literature in individuals with CHRNB1-related disease. Algorithms developed to predict the effect of missense changes on protein structure and function do not agree on the potential impact of this missense change (SIFT: "Tolerated"; PolyPhen-2: "Probably Damaging"; Align-GVGD: "Class C0"). In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
