Uncertain significance for X-linked intellectual disability, Cantagrel type — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_001008537.3(NEXMIF):c.1789A>G (p.Thr597Ala), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the NEXMIF gene (transcript NM_001008537.3) at coding-DNA position 1789, where A is replaced by G; at the protein level this means replaces threonine at residue 597 with alanine — a missense variant. Submitter rationale: This sequence change replaces threonine with alanine at codon 597 of the KIAA2022 protein (p.Thr597Ala). The threonine residue is weakly conserved and there is a small physicochemical difference between threonine and alanine. This variant is not present in population databases (ExAC no frequency). This variant has not been reported in the literature in individuals with KIAA2022-related disease. Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be tolerated, but these predictions have not been confirmed by published functional studies and their clinical significance is uncertain. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
