Pathogenic for Hereditary cancer-predisposing syndrome — the classification assigned by Ambry Genetics to NM_007294.4(BRCA1):c.1045G>T (p.Glu349Ter), citing Ambry Variant Classification Scheme 2023. This variant lies in the BRCA1 gene (transcript NM_007294.4) at coding-DNA position 1045, where G is replaced by T; at the protein level this means converts the codon for glutamic acid at residue 349 into a premature stop signal — a nonsense variant expected to truncate the protein. Submitter rationale: The p.E349* pathogenic mutation (also known as c.1045G>T), located in coding exon 9 of the BRCA1 gene, results from a G to T substitution at nucleotide position 1045. This changes the amino acid from a glutamic acid to a stop codon within coding exon 9. This alteration was identified in a large, worldwide study of BRCA1/2 mutation positive families (Rebbeck TR et al. Hum Mutat, 2018 May;39:593-620). This alteration was also identified in a cohort of Russian ovarian cancer patients (Kechin A et al. Breast Cancer Res Treat, 2023 Jan;197:387-395). This variant is considered to be rare based on population cohorts in the Genome Aggregation Database (gnomAD). In addition to the clinical data presented in the literature, this alteration is expected to result in loss of function by premature protein truncation or nonsense-mediated mRNA decay. As such, this alteration is interpreted as a disease-causing mutation.
