Likely pathogenic for Cystic fibrosis — the classification assigned by Ambry Genetics to NM_000492.4(CFTR):c.3140-1G>A, citing Ambry Variant Classification Scheme 2023. This variant lies in the CFTR gene (transcript NM_000492.4) at the canonical splice acceptor site of the intron immediately before coding-DNA position 3140, where G is replaced by A; at the protein level this means a change at this position may disrupt normal splicing. Submitter rationale: The c.3140-1G>A intronic variant results from a G to A substitution one nucleotide upstream from coding exon 20 of the CFTR gene. Variants that disrupt the canonical splice site are expected to result in aberrant splicing. In silico splice site analysis predicts that this alteration will weaken the native splice acceptor site and may result in the creation or strengthening of a novel splice acceptor site. A resulting transcript is predicted to be in-frame and is not expected to trigger nonsense-mediated mRNAdecay; although, direct evidence is unavailable. However, the region predicted to be impacted is critical for protein function (Ambry internal data). This variant has been identified in the homozygous state and/or in conjunction with other CFTR variant(s) in individual(s) with features consistent with cystic fibrosis (Mercier B et al. Genomics, 1993 Apr;16:296-7). Of note, this alteration is also known as 3272-1G>A in published literature. This nucleotide position is highly conserved in available vertebrate species. Based on the majority of available evidence to date, this variant is likely to be pathogenic.
