This variant lies in the CFTR gene (transcript NM_000492.4) at coding-DNA position 254, where G is replaced by T; at the protein level this means replaces glycine at residue 85 with valine — a missense variant. Submitter rationale: Variant summary: CFTR c.254G>T (p.Gly85Val) results in a non-conservative amino acid change in the encoded protein sequence. Algorithms developed to predict the effect of missense changes on protein structure and function all suggest that this variant is likely to be disruptive. The variant was absent in 250880 control chromosomes. c.254G>T has been observed in the presumed compound heterozygous state in at least 2 individual(s) affected with Cystic Fibrosis (example, Casals_1997, Trujillano_2013). These data indicate that the variant may be associated with disease. A different variant affecting the same codon has been classified as likely pathogenic/pathogenic by our lab (c.254G>A, p.Gly85Glu), supporting the critical relevance of codon 85 to CFTR protein function. At least one publication reports experimental evidence evaluating an impact on protein function. The most pronounced variant effect resulted in approximately 2.30% of normal chloride channel conductance relative to wild type (e.g., Bihler_2024).
