NM_001035.3(RYR2):c.7156A>G (p.Asn2386Asp) was classified as Uncertain significance for Catecholaminergic polymorphic ventricular tachycardia 1 by Labcorp Genetics (formerly Invitae), Labcorp, citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces asparagine with aspartic acid at codon 2386 of the RYR2 protein (p.Asn2386Asp). The asparagine residue is highly conserved and there is a small physicochemical difference between asparagine and aspartic acid. This variant is not present in population databases (ExAC no frequency). This variant has not been reported in the literature in individuals with RYR2-related disease. Algorithms developed to predict the effect of missense changes on protein structure and function do not agree on the potential impact of this missense change (SIFT: "Deleterious"; PolyPhen-2: "Benign"; Align-GVGD: "Class C15"). A different missense substitution at this codon (p.Asn2386Ile) has been reported to segregate with arrhythmogenic right ventricular dysplasia/cardiomyopathy in a large multigenerational family (PMID: 11159936). In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
