Uncertain significance for Epileptic encephalopathy — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_001036.6(RYR3):c.12873C>G (p.Ser4291Arg), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the RYR3 gene (transcript NM_001036.6) at coding-DNA position 12873, where C is replaced by G; at the protein level this means replaces serine at residue 4291 with arginine — a missense variant. Submitter rationale: This variant has not been reported in the literature in individuals with RYR3-related disease. Algorithms developed to predict the effect of missense changes on protein structure and function do not agree on the potential impact of this missense change (SIFT: "Deleterious"; PolyPhen-2: "Benign"; Align-GVGD: "Class C0"). Algorithms developed to predict the effect of sequence changes on RNA splicing suggest that this variant may create or strengthen a splice site, but this prediction has not been confirmed by published transcriptional studies. This sequence change replaces serine with arginine at codon 4291 of the RYR3 protein (p.Ser4291Arg). The serine residue is weakly conserved and there is a moderate physicochemical difference between serine and arginine. This variant is not present in population databases (ExAC no frequency). In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
