NM_000512.5(GALNS):c.1164C>A (p.Asp388Glu) was classified as Pathogenic for Mucopolysaccharidosis, MPS-IV-A by Labcorp Genetics (formerly Invitae), Labcorp, citing Invitae Variant Classification Sherloc (09022015). This variant lies in the GALNS gene (transcript NM_000512.5) at coding-DNA position 1164, where C is replaced by A; at the protein level this means replaces aspartic acid at residue 388 with glutamic acid — a missense variant. Submitter rationale: This sequence change replaces aspartic acid, which is acidic and polar, with glutamic acid, which is acidic and polar, at codon 388 of the GALNS protein (p.Asp388Glu). This variant is present in population databases (rs752339162, gnomAD 0.003%). This missense change has been observed in individual(s) with mucopolysaccharidosis type IVA (Invitae). In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant. It has also been observed to segregate with disease in related individuals. ClinVar contains an entry for this variant (Variation ID: 528322). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) has been performed at Invitae for this missense variant, however the output from this modeling did not meet the statistical confidence thresholds required to predict the impact of this variant on GALNS protein function. For these reasons, this variant has been classified as Pathogenic.
