Uncertain significance for Neuronal ceroid lipofuscinosis — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_006493.4(CLN5):c.574T>G (p.Phe192Val), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the CLN5 gene (transcript NM_006493.4) at coding-DNA position 574, where T is replaced by G; at the protein level this means replaces phenylalanine at residue 192 with valine — a missense variant. Submitter rationale: This sequence change replaces phenylalanine with valine at codon 241 of the CLN5 protein (p.Phe241Val). The phenylalanine residue is highly conserved and there is a small physicochemical difference between phenylalanine and valine. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be disruptive, but these predictions have not been confirmed by published functional studies and their clinical significance is uncertain. This variant has not been reported in the literature in individuals with CLN5-related disease. This variant is not present in population databases (ExAC no frequency).
