Uncertain significance for Hereditary cancer-predisposing syndrome — the classification assigned by Ambry Genetics to NM_000251.3(MSH2):c.373C>A (p.Pro125Thr), citing Ambry Variant Classification Scheme 2023. This variant lies in the MSH2 gene (transcript NM_000251.3) at coding-DNA position 373, where C is replaced by A; at the protein level this means replaces proline at residue 125 with threonine — a missense variant. Submitter rationale: The p.P125T variant (also known as c.373C>A), located in coding exon 3 of the MSH2 gene, results from a C to A substitution at nucleotide position 373. The proline at codon 125 is replaced by threonine, an amino acid with highly similar properties. In a massively parallel cell-based functional assay testing susceptibility to a DNA damaging agent, 6-thioguanine (6-TG), this variant was determined to be functionally neutral (Jia X et al. Am J Hum Genet, 2021 Jan;108:163-175). This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Based on the available evidence, the clinical significance of this variant remains unclear.
