Uncertain significance for Primary ciliary dyskinesia — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_001256715.2(DNAAF3):c.-69C>G, citing Invitae Variant Classification Sherloc (09022015). This variant lies in the DNAAF3 gene (transcript NM_001256715.2) at 69 bases upstream of the translation start (5' untranslated region), where C is replaced by G. Submitter rationale: This variant is present in population databases (rs772986651, ExAC 0.001%). This variant has not been reported in the literature in individuals with DNAAF3-related disease. Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be tolerated, but these predictions have not been confirmed by published functional studies and their clinical significance is uncertain. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. This sequence change replaces proline with arginine at codon 21 of the DNAAF3 protein (p.Pro21Arg). The proline residue is weakly conserved and there is a moderate physicochemical difference between proline and arginine.
