Uncertain significance for Hereditary cancer-predisposing syndrome — the classification assigned by Ambry Genetics to NM_004360.5(CDH1):c.437C>G (p.Ser146Cys), citing Ambry Variant Classification Scheme 2023. This variant lies in the CDH1 gene (transcript NM_004360.5) at coding-DNA position 437, where C is replaced by G; at the protein level this means replaces serine at residue 146 with cysteine — a missense variant. Submitter rationale: The p.S146C variant (also known as c.437C>G), located in coding exon 4 of the CDH1 gene, results from a C to G substitution at nucleotide position 437. The serine at codon 146 is replaced by cysteine, an amino acid with dissimilar properties. This variant was not reported in population based cohorts in the following databases: Database of Single Nucleotide Polymorphisms (dbSNP), NHLBI Exome Sequencing Project (ESP), and 1000 Genomes Project. In the ESP, this variant was not observed in 6498 samples (12996 alleles) with coverage at this position. To date, this alteration has been detected with an allele frequency of approximately 0.001% (greater than 175000 alleles tested) in our clinical cohort. This amino acid position is not conserved beyond primates. In addition, this alteration is predicted to be tolerated by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of p.S146C remains unclear.
