Pathogenic for Hereditary cancer-predisposing syndrome; Familial thoracic aortic aneurysm and aortic dissection — the classification assigned by Ambry Genetics to NM_005359.6(SMAD4):c.1308+1G>A, citing Ambry Variant Classification Scheme 2023. This variant lies in the SMAD4 gene (transcript NM_005359.6) at the canonical splice donor site of the intron immediately after coding-DNA position 1308, where G is replaced by A; at the protein level this means a change at this position may disrupt normal splicing. Submitter rationale: The c.1308+1G>A intronic pathogenic mutation results from a G to A substitution one nucleotide after coding exon 9 of the SMAD4 gene. Using the BDGP and ESEfinder splice site prediction tools, this alteration is predicted to abolish the native splice donor site; however, direct evidence is unavailable. This mutation has been identified in an individual satisfying clinical diagnostic criteria for juvenile polyposis syndrome (JPS) (Ambry internal data). In addition to this data, alterations that disrupt the canonical splice site are expected to cause aberrant splicing, resulting in an abnormal protein or a transcript that is subject to nonsense-mediated mRNA decay. As such, this alteration is classified as a disease-causing mutation.
