Uncertain significance for Cardiovascular phenotype — the classification assigned by Ambry Genetics to NM_000335.5(SCN5A):c.251A>T (p.Asp84Val), citing Ambry Variant Classification Scheme 2023. This variant lies in the SCN5A gene (transcript NM_000335.5) at coding-DNA position 251, where A is replaced by T; at the protein level this means replaces aspartic acid at residue 84 with valine — a missense variant. Submitter rationale: The p.D84V variant (also known as c.251A>T), located in coding exon 1 of the SCN5A gene, results from an A to T substitution at nucleotide position 251. The aspartic acid at codon 84 is replaced by valine, an amino acid with highly dissimilar properties. This variant was not reported in population based cohorts in the following databases: Database of Single Nucleotide Polymorphisms (dbSNP), NHLBI Exome Sequencing Project (ESP), and 1000 Genomes Project. In the ESP, this variant was not observed in 6090 samples (12180 alleles) with coverage at this position. This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear.
