Pathogenic for Hereditary cancer-predisposing syndrome — the classification assigned by Ambry Genetics to NM_000038.6(APC):c.1626+1G>A, citing Ambry Variant Classification Scheme 2023. This variant lies in the APC gene (transcript NM_000038.6) at the canonical splice donor site of the intron immediately after coding-DNA position 1626, where G is replaced by A; at the protein level this means a change at this position may disrupt normal splicing. Submitter rationale: The c.1626+1G>A intronic pathogenic mutation results from a G to A substitution one nucleotide after coding exon 12 of the APC gene. This alteration has been observed in individuals with a personal and/or family history that is consistent with APC-related disease (Ambry internal data). RNA studies have demonstrated that this alteration results in abnormal splicing in the set of samples tested (Ambry internal data). This nucleotide position is highly conserved in available vertebrate species. In silico splice site analysis predicts that this alteration will weaken the native splice donor site. This variant is considered to be rare based on population cohorts in the Genome Aggregation Database (gnomAD). Alterations that disrupt the canonical splice site are expected to cause aberrant splicing, resulting in an abnormal protein or a transcript that is subject to nonsense-mediated mRNA decay. As such, this alteration is classified as a disease-causing mutation.
