Uncertain significance for Microcephaly, normal intelligence and immunodeficiency — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_002485.5(NBN):c.1660G>T (p.Asp554Tyr), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the NBN gene (transcript NM_002485.5) at coding-DNA position 1660, where G is replaced by T; at the protein level this means replaces aspartic acid at residue 554 with tyrosine — a missense variant. Submitter rationale: This sequence change replaces aspartic acid with tyrosine at codon 554 of the NBN protein (p.Asp554Tyr). The aspartic acid residue is weakly conserved and there is a large physicochemical difference between aspartic acid and tyrosine. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. Algorithms developed to predict the effect of sequence changes on RNA splicing suggest that this variant may create or strengthen a splice site, but this prediction has not been confirmed by published transcriptional studies. Algorithms developed to predict the effect of missense changes on protein structure and function are either unavailable or do not agree on the potential impact of this missense change (SIFT: "Tolerated"; PolyPhen-2: "Possibly Damaging"; Align-GVGD: "Class C0"). This variant has not been reported in the literature in individuals with NBN-related conditions. ClinVar contains an entry for this variant (Variation ID: 492099). This variant is not present in population databases (ExAC no frequency).
