NM_004656.4(BAP1):c.255+1G>A was classified as Uncertain significance for Hereditary cancer-predisposing syndrome by Ambry Genetics, citing Ambry Variant Classification Scheme 2023. This variant lies in the BAP1 gene (transcript NM_004656.4) at the canonical splice donor site of the intron immediately after coding-DNA position 255, where G is replaced by A; at the protein level this means a change at this position may disrupt normal splicing. Submitter rationale: The c.255+1G>A intronic variant results from a G to A substitution one nucleotide after coding exon 4 of the BAP1 gene. This alteration was non-functional in a high throughput genome editing haploid cell survival functional assay (Waters AJ et al. Nat Genet, 2024 Jul;56:1434-1445). Alterations that disrupt the canonical splice site are expected to result in aberrant splicing. In silico splice site analysis predicts that this alteration will weaken the native splice donor site. A resulting transcript is predicted to be in-frame and is not expected to trigger nonsense-mediated mRNAdecay; although, direct evidence is unavailable. This nucleotide position is highly conserved in available vertebrate species. Based on the available evidence, the clinical significance of this variant remains unclear.
