This variant lies in the BLM gene (transcript NM_000057.4) at coding-DNA position 205, where G is replaced by T; at the protein level this means converts the codon for glutamic acid at residue 69 into a premature stop signal — a nonsense variant expected to truncate the protein. Submitter rationale: The E69X variant in the BLM gene has not been reported previously as a pathogenic variant nor as a benign variant, to our knowledge. This variant is predicted to cause loss of normal protein function either through protein truncation or nonsense-mediated mRNA decay. The E69X variant is not observed in large population cohorts (Lek et al., 2016). We interpret E69X as a pathogenic variant.
