This variant lies in the IL12RB1 gene (transcript NM_005535.3) at coding-DNA position 913, where A is replaced by T; at the protein level this means converts the codon for lysine at residue 305 into a premature stop signal — a nonsense variant expected to truncate the protein. Submitter rationale: The K305X variant in the IL12RB1 gene has been reported previously in he homozygous state in a patient with disseminated mycobacterial infection attributable to BCG; flow cytometry studies on T-cells from this individual suggested that this variant may preclude surface expression of the receptor (Altare et al., 1998). This variant is predicted to cause loss of normal protein function either through protein truncation or nonsense-mediated mRNA decay. The K305X variant is not observed in large population cohorts (Lek et al., 2016). We interpret K305X as a pathogenic variant.
