NM_000054.7(AVPR2):c.575G>T (p.Cys192Phe) was classified as Pathogenic for Diabetes insipidus, nephrogenic, X-linked by Department of Traditional Chinese Medicine, Fujian Provincial Hospital, citing ACMG Guidelines, 2015: AVPR2 NM_000054.7:c.575G>T (p.Cys192Phe) was detected hemizygously in a male patient with a phenotype highly consistent with X-linked nephrogenic diabetes insipidus and was confirmed by Sanger sequencing. Cys192 is a conserved cysteine in the second extracellular loop of V2R and is predicted to form a critical disulfide bond with Cys112. Functional studies of other substitutions at Cys192 have shown impaired V2R function/dDAVP-induced cAMP signaling, and another missense change at the same residue, p.Cys192Arg, has been reported in NDI patients. The variant is absent or extremely rare in population databases and has deleterious in silico prediction support (REVEL 0.669). Classified as Pathogenic according to ACMG/AMP criteria: PS2 or PM6, PM1, PM2_supporting, PM5, PP3, and PP4.
