NM_000203.5(IDUA):c.469T>C (p.Ser157Pro) was classified as Uncertain Significance for Mucopolysaccharidosis type 1 by ClinGen Lysosomal Storage Disorder Variant Curation Expert Panel, citing ClinGen LSD ACMG Specifications IDUA V1.2.0: The NM_000203.5:c.469T>C (p.Ser157Pro) variant in IDUA is a missense variant resulting in the substitution of serine by proline at amino acid 157. This variant has been reported in at least five individuals with MPS Type 1. In one patient, the variant was seen in conjunction with the pathogenic variant c.1A>C, with phase confirmed through parental testing (PMID: 31236806). The variant has also been detected in the homozygous state in at least three other patients (PMID: 31386236). Of note, in one case (PMID: 35005816) the patient was homozygous for the c.469T>C variant, but also the likely pathogenic variant c.523_543del. The presence of this alternative molecular explanation raises suspicion that the in-frame deletion is the disease-causing variant, and that technical limitations on sequencing may have precluded this in cis disease-causing variant from being detected in other cases. There was insufficient phenotypic specificity reported in the cases to apply the PP4 criterion. The variant is absent from gnomAD v4.1.0 (PM2_Supporting). The computational predictor REVEL gives a score of 0.618, which is insufficient for application of PP3. Therefore, at this stage, this variant meets the criteria to be classified as a variant of uncertain significance for MPS I based on the IDUA-specific ACMG/AMP criteria applied, as specified by the ClinGen Lysosomal Diseases Variant Curation Expert Panel (Specifications Version 1.2.0): PM2_Supporting (Classification approved by the ClinGen Lysosomal Diseases Variant Curation Expert Panel, April 6, 2026)
